You can get benefits from each membership level

• The medical check-up benefits cannot be assigned/transferred, it can only be used by the name listed on the Medical Check Up Reservation Form and proven by an Identity Card document (ID Card/Family Card).

• Reservations are made no later than H-5 through Loyalty Platform.

• eVoucher code/medical check up guarantee letter and Identity Card (original KTP / KK) must be shown to the intended Health Facility Officer at the time of the medical check up.

• Medical check-up operational time is the operational hours according to each Health Facility Location.

• If the reservation is made on Sunday or National Holiday, then the reservation cannot be served.

• Customers must fast 10-12 hours before the medical check-up.

• eVoucher Code/Bank Guarantee Letter can only be used once and is valid for a maximum of 7 working days from the date of medical check-up stated in the reservation form.

• The eVoucher code/Bank Guarantee Letter will be considered forfeited and cannot be used again if a maximum of 7 working days from the specified date the customer does not attend the selected health facility.

• To change/reschedule the medical check-up schedule, the Customer must reapply at Loyalty Platform.
