• The medical check-up benefits cannot be assigned/transferred, it can only be used by the name listed on the Medical Check Up Reservation Form and proven by an Identity Card document (Passport/Family Card).

• Reservations are made no later than H-5 through Loyalty Platform.

• Reservation is successful if you have received confirmation from Loyalty Platform via email.

• Medical check-up operational time is the operational hours according to each Health Facility Location.

• If the reservation is made on Sunday or National Holiday, the reservation cannot be served.

• Customers are required to do fasting 10-12 hours before the medical check-up.
