Customers are required to fill out and sign the following reservation form:
1.Parkway Cancer Centre
2.Mount Elizabeth Hospital
3.Mount Elizabeth Novena Hospital
4.Gleneagles Hospital
5.Parkway East Hospital

Specifically for Parkway Cancer Centre collaboration, the following conditions apply:

2.Get a 10% discount on consultations with a cancer specialist. Privileges apply only to doctor consultation fees (for the first consultation and one follow-up consultation). This does not include hospitalization, medication, injections, surgery, special procedures, laboratory tests, or other supporting examinations outside the scope of the consultation. The following consultation fees apply at Parkway Cancer Center. Download here
3.Customers can also make reservations directly by showing the reservation form via WA to Parkway Cancer Center, Jakarta WA +62 811 1934 673
4.The reservation form that has been referenced and signed by Canhope must be brought when visiting the intended Health Facility.

First Email
"Thank you for being a loyal BTN Private customer. Your international Medical Concierge reservation is currently being processed."
Second Email
"Thank you for your trust as a BTN Private customer. Your International Medical Concierge reservation has been successfully confirmed. Please bring your referral letter with you to your medical check-up."
PLUS Assistant provides confirmation to the Customer via email regarding information from the PIC.
